As you may know, plan sponsors are required to provide Medicare Part D creditable and non-creditable notifications. We're pleased to inform you that, as part of our ongoing commitment to supporting clients, Prime now has the simple determination results (PASS/FAIL) available for each of your plans. Please contact your Prime account executive if you'd like to access these results.   

Model Notice Letters  

To further assist you in this process, we encourage you to refer to the Centers for Medicare & Medicaid Services (CMS) template letters for member mailings. These Model Notice Letters are available in both English and Spanish for creditable and non-creditable coverage.   

Disclosure notice to CMS  

Additionally, we’d like to remind you about the disclosure notice to CMS. Please note that the only way to comply with the CMS disclosure requirement is by completing the online “Disclosure to CMS Form” on the CMS Creditable Coverage Disclosure web page.
