Rights and responsibilities

As a member of our patient management program, you have the right to: 

  • Receive information about the Care Specialists program 
  • Know the identity of your care specialist and their clinical qualifications and speak with a supervisor or pharmacist if you request
  • Decline participation or withdraw from the program 
  • Have your treatment and other consumer information kept confidential; only where permitted by law may records be released without your permission 
  • Receive notification regarding changes in or termination of the Care Specialists program 
  • Have personal health information shared with the patient management program, only in accordance with state and federal law 
  • Speak to a health professional 

As a member of our patient management program, you have the responsibility to: 

  • Submit any forms that are necessary to participate in the program, to the extent required by law 
  • Give accurate clinical and contact information and notify your health coach with any changes 
  • Notify your treating provider of your participation in the Care Specialists program
