What you need to know 

As you may know, plan sponsors are required to provide Medicare Part D Creditable and Non-Creditable Notifications. We're pleased to inform you that, as part of our ongoing commitment to support clients, Prime will be providing you with the Simple Determination result (PASS/FAIL) for each of your plans.

Model Notice Letters 

To assist you further in this process, we'd like to direct your attention for reference to Centers for Medicare & Medicaid Services (CMS) template letters for member mailings. These Model Notice Letters are available in both English and Spanish for Creditable and Non-Creditable coverage.  

Disclosure Notice to CMS 

Additionally, we want to remind you about the Disclosure Notice to CMS. Please note that the only way to comply with the CMS disclosure requirement is by completing the online CMS Form ("Disclosure Form") on the CMS Creditable Coverage Disclosure web page.
