Program identified millions in health care savings when pharmacists help members optimize drug therapy

Prime and a Blue Plan client together developed a program to helped Medicare and commercial members with certain health conditions optimize their drug therapy. Pharmacists reviewed claims to identify high risk and high cost members. The pharmacists worked with care teams and conducted outreach. The Blue Plan verified a 10:1 ROI from the program.
