Decision effective dates are intended for the first day of each quarter following the P&T meeting. These dates are subject to change.
Custom formulary decisions are typically made two to six weeks following the National P&T decisions according to the same schedule.
Chapters are determined using the Medi-Span classification system with a few exceptions.
