Medication Therapy Management Program
Prime is committed to optimizing therapeutic outcomes through improved medication use, as well as reducing the risk of adverse drug events. While many of Prime's programs (Formulary Development, Care Management, Concurrent Drug Utilization Review and others) contribute to this goal, Prime believes it is prudent to implement a focused Medication Therapy Management Program (MTMP) for the Medicare Part D programs, given the unique needs and increased vulnerability of this population.
Prime, in consultation with our clients, physicians, and licensed and practicing pharmacists will implement an MTMP designed to optimize therapeutic outcomes for targeted beneficiaries. Prime will employ a two-pronged approach to MTMP - centralized and local/retail - by contracting with pharmacy network providers along with directly providing consultative services to achieve one or more of the following goals:
- Enhance member understanding through education counseling that promotes the appropriate use of medications and reduces the risk of potentially adverse events associated with the use of medications
- Increase member adherence to prescription medication regimens
- Detect potential adverse drug events and patterns of over use and under use of prescription drugs
