2008 MEDICARE PART D FORMULARIES
Attention Pharmacists: There are significant changes to the Medicare Part D formularies in 2008 due to CMS mandates and a migration to one standard formulary for most of our plans. Member letters will be mailed in November 2007. We ask pharmacists to work with members and physicians in order to make this a smooth transition. Coverage determinations for changes can be submitted by the prescribing physician after December 17, 2007 for an effective date of 1/1/08.
